Post Game/Post-Competition Release Form

Complete this form in order for your child to be released to ride home with you after a game or a competition. **Note:** This form must be approved by Mr. Trobaugh and delivered to the Bus Chaperone BEFORE DEPARTURE from the Munford High School Campus.

Student Travel Permission Form & Website Pictures Permission Form

This document contains 2 forms on one page. One form grants your child permission to travel with the band to all events (games, competitions, etc.) The other form is to allow or deny pictures taken of your child to be placed on the website at munfordband.com.

Medical Release Form

This form must be completed and submitted to the Band Director or Chaperone Chairman before your child can go to any football game or band competition.
